On the front of the mouse, there are two quite large buttons. These may be configured to your preferences, or you can just use them as normal left and right-click buttons. It has an ambidextrous design and is quite pleasant to use.

I own two of these, one for each hand, and it takes some getting used to, but it is worth it. It's easy to abuse these and use these and scroll with one finger like a regular mouse, but if you use them with proper form and distribute the rolling motion of the ball evenly with all of your hand, instead of just one finger like a regular mouse scroll wheel, then it will alleviate hand/wrist pain.
